How a strain affects muscle
A rapid acceleration, forceful stretch, awkward lift, or load beyond current capacity can disrupt muscle fibres. Symptoms usually centre on the involved muscle and become clearer when it contracts or lengthens. A strain may also present with spasm, reduced power, or a limited range of movement. Delayed soreness after unfamiliar exercise is more diffuse and often begins later, while a strain is commonly linked to a specific movement or moment of pain.
The examination checks tenderness, strength, movement, and the nearby joint or tendon. Marked weakness, a visible gap, or loss of normal function may indicate a larger injury and can change management.
Protect, move, and rebuild
Stop the activity that caused sharp pain and protect the area from another sudden load. During the early phase, comfortable movement and light use are generally preferable to repeatedly testing the injury or keeping it completely still. A wrapped cold pack may ease pain; place fabric between the pack and skin and avoid using it on numb skin. Compression or elevation can be useful when swelling is present, provided circulation and sensation remain normal.
As walking or daily movement becomes comfortable, rebuild range, strength, and task-specific loading gradually. Do not use pain relief to push through sprinting, heavy lifting, or explosive exercise. A physiotherapist can guide progression when the injured muscle is essential for sport or work, or when weakness persists.
When an examination should not wait
Get urgent help if the limb becomes cold, pale, blue, numb, increasingly tense, or extremely painful, or if there is major deformity or an open wound. Prompt assessment is also appropriate when you heard a snap, cannot bear weight or use the muscle, have rapidly expanding swelling or bruising, or develop calf swelling with breathlessness or chest pain.

When can I return to exercise after a strain?
