The short answer

Treatment is usually organised in hospital and can also include pain relief, fluids, temporary fasting and assessment for gallbladder surgery or drainage. The antibiotic choice depends on illness severity, allergies, kidney function and local clinical guidance.

Why antibiotics are used

Acute cholecystitis is inflammation of the gallbladder, commonly after a stone blocks its outlet. Trapped bile and tissue injury can allow bacterial infection to develop. Antibiotics are given when infection is suspected or when the risk of infectious complications is clinically significant.

Improvement in fever or pain does not show that the obstruction has cleared. Clinicians review examination findings, blood results and ultrasound imaging to decide whether definitive gallbladder treatment is needed.

Using treatment safely

Take prescribed antibiotics exactly as directed and tell the treating team about previous reactions, pregnancy, kidney problems and all current medicines. Do not use leftover tablets or another person’s prescription, because the drug and dose may not suit the likely bacteria or your health.

Report a new rash, facial swelling, breathing difficulty or severe persistent diarrhoea promptly. Keep follow-up appointments even when symptoms settle, since the underlying gallbladder problem may still require a procedure.

Warning signs needing urgent care

Seek urgent assessment for steady or worsening pain in the upper right abdomen, fever, repeated vomiting, yellow skin or eyes, dark urine, faintness or confusion. These features can accompany significant inflammation, a blocked bile duct or spreading infection.

Call emergency services for collapse, severe breathing difficulty or rapidly increasing drowsiness. If symptoms worsen while taking antibiotics, do not wait until the course is finished before returning for care.

Ask whether infection is suspected, how long the prescribed course should continue, what side effects need attention and whether a culture may change the medicine. Clarify the plan for surgery or drainage and what you may eat or drink while awaiting treatment.
