The short answer

Some people feel well despite liver inflammation, while others develop fatigue, nausea, reduced appetite, discomfort on the right side of the upper abdomen, dark urine or yellowing of the eyes and skin. Symptoms alone cannot show how active the infection is.

What an active infection means

Clinicians interpret blood tests for hepatitis B antigens and antibodies alongside the amount of viral genetic material in the blood. Liver enzyme tests help show whether liver cells are irritated, but a single result does not give the whole picture. Results over time, medical history and sometimes liver imaging or an assessment of liver stiffness guide decisions.

Active infection does not automatically mean severe liver damage. It does mean the virus can require follow-up and can pass through blood or certain body fluids. Household and sexual contacts may need testing and vaccination if they are not already protected.

What to do next

Arrange a clinical review to confirm which markers are positive, assess liver health and decide whether monitoring alone or antiviral treatment is appropriate. Keep scheduled blood tests and imaging because changes can occur without obvious symptoms. Do not stop prescribed antiviral medicine suddenly; viral activity can rebound and inflame the liver.

Avoid alcohol and check medicines, supplements and herbal products with a clinician or pharmacist before taking them. Do not share razors, toothbrushes, needles or equipment that may carry blood. Cover open cuts, use barrier protection until partners have been assessed, and never donate blood. Hepatitis B is not spread by hugging, sharing meals, coughing or ordinary social contact.

When to seek urgent care

Seek urgent medical assessment for new yellowing of the skin or eyes, repeated vomiting, severe or increasing abdominal pain, unusual bleeding, marked swelling of the abdomen, confusion, extreme sleepiness or difficulty staying awake. These features can signal significant liver dysfunction and should not wait for a routine appointment.

Contact your clinician promptly if dark urine, pale stools, worsening fatigue or loss of appetite develops, or if you cannot take prescribed medicine. After a possible recent exposure to infected blood or body fluids, obtain medical advice promptly because time-sensitive preventive care may be considered.
