How active hand therapy works

Unlike passive movement, where a therapist or the other hand moves a joint, active exercise asks the healing muscles and tendons to produce the motion. The programme may include tendon-gliding patterns, controlled wrist or finger movement, grip tasks, dexterity practice, swelling control, scar care, and gradual return to work or household activities.

Why exercises must match the diagnosis

The safe movement range differs after a distal radius fracture, tendon repair, carpal tunnel release, Dupuytren's surgery, or an arthritis flare. Healing tissue may need protection even when the hand feels capable of more. A hand therapist therefore bases exercise on the operation or injury, tissue-healing stage, swelling, sensation, pain response, and the surgeon's precautions.

The purpose is functional recovery, not simply completing repetitions. Buttoning clothing, turning a key, typing, lifting a cup, or handling tools can reveal limitations in coordination and endurance that a basic range check may miss.

Getting the most from a programme

Follow the prescribed movement, frequency, and resistance rather than adding force independently. Perform exercises with controlled technique and note how the hand responds later that day. Mild stretching or effort can occur, but sharp pain, increasing swelling, catching, or a lasting loss of movement signals that the plan needs adjustment.

Bring splints and exercise equipment to reviews so their fit and use can be checked. Tell the therapist which everyday tasks matter most to you; therapy can then rehearse the grip, reach, speed, or endurance those activities require. Protect wounds and follow any lifting or movement restrictions from the surgical team.

When to stop and get help

Pause the exercise and contact the care team for a sudden loss of movement, a snapping sensation after tendon surgery, increasing wound drainage, spreading redness, fever, or a splint that causes sores or persistent numbness. Urgent assessment is needed if fingers become pale, blue, cold, increasingly numb, or extremely swollen, or if severe pain does not ease after removing a tight removable wrap as instructed.

Questions before progressing

Clarify which structures are still healing, what movement and lifting limits apply, whether the splint may be removed for exercise, how much discomfort is acceptable, which signs mean the exercise should stop, and what functional milestone must be reached before driving, work, sport, or heavier gripping.

Should active hand exercises hurt?

Some effort or a gentle stretch may be expected, but sharp pain, increasing swelling, new catching, or worse movement afterward should be reported to the therapist.

Can I use online hand exercises instead of a tailored programme?

Generic exercises may conflict with surgical precautions or the tissue that is healing. An individualized plan is safer when there has been injury, surgery, nerve trouble, or significant inflammation.
