What should you know about actinic lichen planus face?

Patches may look brown, grey-brown or violet and can occur on the forehead, cheeks or other exposed areas. Facial skin needs carefully selected treatment because it is more vulnerable to irritation and certain medicine side effects.

The visible colour may come from active inflammation, pigment left after inflammation, or both. The rash may be smooth or slightly raised, and itch is not required for the diagnosis. Its pattern can overlap with melasma, contact reactions and other inflammatory pigment disorders.

Your dermatologist may examine the scalp, mouth, nails and the rest of the skin for features of lichen planus. A close examination and sometimes a small biopsy help separate similar-looking conditions before a facial treatment plan is chosen.

Use shade, a hat and broad-spectrum sunscreen consistently, including around the hairline and sides of the face. Choose gentle cleanser and moisturiser, and pause new acids, scrubs or fragranced products if they sting or redden the area.

Do not apply a potent steroid or bleaching cream to the face unless it was prescribed for that exact site. A dermatologist can match anti-inflammatory treatment to the skin area and monitor for thinning, acne-like eruptions or colour change. Improvement in inflammation may occur before the remaining pigment fades.

Book an assessment if facial patches are new, enlarging, changing texture or not responding to gentle care. Obtain quicker help for swelling around the eyes, eye pain, vision changes, widespread blistering, fever or rapidly increasing tenderness. Those symptoms are not explained by simple pigmentation and need prompt evaluation.

Confirm whether the patches show active inflammation or mainly leftover pigment, and ask whether dermoscopy or biopsy would change management. Check precisely how much medicine to use, which facial zones to avoid, how to combine it with sunscreen, and when progress should be reassessed.
