Is actinic keratosis premalignant?

Premalignant does not mean that a person already has skin cancer or that a particular spot will inevitably become cancerous. It describes abnormal cells confined to the upper skin that carry potential for further change. A clinician cannot reliably predict the future of every individual patch just by looking at it.

Actinic keratoses often occur within a broader area altered by cumulative ultraviolet exposure. That field may contain visible rough spots as well as less obvious cellular damage, which is why the surrounding skin and other exposed sites are examined too.

Book a skin assessment to confirm the diagnosis and discuss whether treatment should target a single lesion or the wider sun-damaged field. A defined patch may be frozen or removed, while multiple lesions may be approached with prescription creams or photodynamic therapy. Selection depends on site, thickness, skin tolerance and the expected healing response.

Reduce further ultraviolet injury with shade, protective clothing and regular use of broad-spectrum sunscreen on exposed skin. Continue self-checks after treatment because clearing one lesion does not reverse the underlying tendency of sun-damaged skin to form new ones.

Request an earlier appointment if a known patch rapidly becomes thicker, forms a firm or tender bump, repeatedly bleeds, ulcerates or keeps returning after treatment. Squamous cell carcinoma can present with these changes, but an examination and sometimes a biopsy are required to determine the cause. New inflammation during a prescribed topical course may instead be an expected treatment reaction; follow the instructions supplied by the treating clinician.

You can ask whether any lesion looks thick enough to sample, whether treatment should cover a field or selected spots, what skin reaction to expect, and which change should trigger an earlier return. Confirm how often your wider skin needs examination based on your findings and history.
