Actinic Keratosis in Outdoor Workers

Regular work in direct daylight can repeatedly expose the face, ears, scalp, neck, forearms, and backs of the hands to ultraviolet radiation. Actinic keratosis may feel like a persistent rough, gritty, or scaly patch before it looks striking. It reflects damage in surface skin cells and can arise within a broader field of sun-damaged skin.

The finding is not the same as invasive skin cancer. However, an individual patch can sometimes develop into squamous cell carcinoma, and a clinician cannot reliably predict that course from texture alone. A full examination is useful because other exposed sites may contain subtle lesions that are hard to see during a self-check.

Combine several barriers: use available shade, wear a broad-brimmed or neck-protecting hat when compatible with safety equipment, choose tightly woven long sleeves, and cover the hands where the task allows. Apply broad-spectrum sunscreen to uncovered skin and reapply according to its label, especially after heavy sweating or wiping the face. Lip balm with ultraviolet protection helps cover an easily missed site.

Discuss practical controls with the workplace, such as shaded rest areas, task rotation away from peak exposure, and uniforms or helmets with compatible sun protection. These measures support rather than replace required protective equipment. Photograph a new patch beside a familiar reference and check whether its thickness, colour, surface, or symptoms change.

Book a skin examination for a persistent new rough patch, particularly on the lip, ear, scalp, face, or hand. Seek an earlier review if the area grows quickly, becomes a firm or raised lump, repeatedly bleeds, ulcerates, or develops increasing pain or tenderness. Urgency follows those changes, not how common a patch may be among outdoor workers.

A dermatologist may examine the whole sun-exposed field and decide whether the diagnosis is clear clinically or a sample is needed. Treatment can target one lesion or a wider damaged area, depending on the number, thickness, location, previous therapy, and ability to protect healing skin while working.

Ask whether treatment will create crusting, open skin, or light sensitivity that affects your usual duties. Clarify what covering is safe over the treated area, when protective equipment can touch it, how to manage sweat and dust, and when the site should be checked again. Mention medicines that suppress immunity and any previous skin cancer.
