Imiquimod for Actinic Keratosis: What to Expect

Actinic keratoses are rough or scaly areas produced by long-term ultraviolet damage. Imiquimod is a prescription cream that activates immune signalling in the application area so abnormal surface cells can be cleared. A dermatologist may choose it when several visible or subtle lesions occupy a wider patch of skin rather than a single isolated spot.

Redness, crusting, flaking, tenderness, itching, or small sores can develop where the medicine is working. The strength of this reaction varies and does not by itself show whether treatment has succeeded. Healthy-looking skin nearby may also react if it lies within the prescribed treatment field.

Apply imiquimod only to the area and on the schedule marked by your prescriber. Wash your hands after application, avoid the eyes, nostrils, mouth, and broken skin unless specifically directed, and do not add extra applications after a missed dose. Protect the treated area from ultraviolet exposure with shade, clothing, and a suitable sunscreen.

Before using moisturiser, dressings, cosmetics, hydrocortisone, or another medicated product on the site, check what your dermatologist permits. Take a clear photograph before treatment and note the application days; this can help the clinician judge whether the reaction matches the intended plan. Do not pick crusts or scrape lesions.

Contact the prescribing clinic promptly if pain is difficult to manage, the skin becomes extensively raw, swelling spreads beyond the application area, pus or fever develops, or the reaction prevents normal sleep or daily activity. The clinician may advise a treatment pause or examine the area; do not change the plan without that guidance.

Arrange reassessment if a lesion persists after the skin has healed, rapidly thickens, forms a firm lump, repeatedly bleeds, or becomes increasingly tender. Those changes need examination because actinic keratosis can sometimes progress to squamous cell carcinoma or may resemble another condition.

Ask exactly which borders to treat, what degree of inflammation is expected, what products may soothe the site, and when photographs or an in-person check are needed. Also ask how the treated field will be assessed after healing and whether individual thicker lesions need a different approach such as freezing or sampling.
