Actinic Cheilitis and Solar Keratosis of the Lip

The terms point to the same central process: repeated ultraviolet exposure damages keratin-producing cells on the lip. The condition often affects a broader, less sharply outlined area than an actinic keratosis on ordinary skin.

Actinic cheilitis can cause persistent dryness, scale, pale or red discoloration, blurring of the border between the lip and nearby skin, or a rough patch. It is considered precancerous because damaged surface cells can sometimes progress to squamous cell carcinoma.

A clinician may diagnose a typical presentation by examination. A focal thickened, ulcerated, bleeding, or indurated area may need a biopsy to determine whether changes remain at the surface or extend more deeply.

Use broad-spectrum sun protection made for lips, reapply it after eating or wiping the mouth, and add shade or a brimmed hat during outdoor activity. Avoid tanning devices and tobacco, which can add further stress to lip tissue.

Treatment depends on whether the change is focal or widespread and on biopsy findings when sampling is needed. Options can include a clinician-prescribed topical medicine, photodynamic therapy, laser resurfacing, or surgical removal of a concerning focus. Some treatments cause temporary inflammation while damaged cells are cleared.

Book a timely examination if roughness, scaling, or colour change persists despite simple lip care. Seek a faster assessment when a spot becomes firm, grows, repeatedly bleeds, forms a sore that does not heal, or causes increasing focal pain.

Do not wait for the whole lip to change. A new focal feature within a background of longstanding sun damage can be the part that needs targeted examination or sampling.

Ask which areas are clinically typical and whether any spot needs biopsy. Discuss whether treatment should cover the whole affected field or only a focal lesion, what reaction to expect during healing, and how future lip checks will be arranged.
