Pathology at a glance

A pathology report may describe thickened or thinned surface tissue, disordered maturation of keratinocytes, atypia, inflammation, and changes in the sun-damaged supporting tissue. These findings help the clinician distinguish actinic cheilitis from look-alike lip disorders and judge whether further treatment is needed.

How to understand common report terms

Atypia or dysplasia means some cells look or mature abnormally; it does not by itself mean invasive cancer. Hyperkeratosis refers to excess surface keratin, while atrophy describes thinning. Solar elastosis is a change in the tissue beneath the surface associated with long-term ultraviolet exposure.

The pathologist also checks whether abnormal cells remain confined to the surface layer or extend deeper. Your dermatologist interprets that microscopic description together with the size, texture, location, and appearance of the lip change.

What to do after a biopsy result

Arrange a follow-up to review the exact diagnosis, the degree and extent of abnormal change, and whether the sampled edges contain the lesion. Management may involve careful monitoring, a prescribed topical medicine, photodynamic therapy, laser resurfacing, or another procedure selected for the clinical and microscopic findings.

Protect the lips with a broad-spectrum lip product and a brimmed hat, and avoid tobacco exposure and picking at crusts. Follow the wound-care instructions for the biopsy site, including guidance on cleansing and any prescribed ointment.

Changes that need prompt review

Seek a prompt dermatology assessment for a lip area that becomes firm or thick, develops a persistent ulcer, bleeds repeatedly, grows, or forms a focal tender lump. These features warrant examination and may lead to a biopsy even when previous changes seemed mild.

After a biopsy, contact the treating clinic for worsening swelling, spreading redness, pus, fever, bleeding that does not settle with steady pressure, or pain that is intensifying rather than easing.

Ask whether the report shows dysplasia, whether deeper abnormal growth was excluded, whether the biopsy sampled the most concerning area, and what treatment fits the result. It is also useful to clarify how the rest of the lip will be monitored and which visible changes should trigger an earlier appointment.
