Combined ACL and MCL tears: diagnosis and treatment

Short answer
A combined ACL and MCL tear damages two stabilisers that control rotation, forward shift and inward opening of the knee.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Answer in brief
The injury can follow a planted-foot twist or a blow that drives the knee inward. Severity varies, so the words ACL and MCL tear do not by themselves determine whether surgery is needed.
How the two ligaments work together
The ACL lies inside the joint and restrains excessive forward travel and rotation of the shin. The MCL spans the inner knee and limits sideways opening. Damage to both can produce instability in more than one direction, particularly during cutting, turning or an unexpected change of balance.
Typical presentations include pain, swelling, inner-side tenderness, loss of movement and episodes of giving way. Those features can also accompany meniscus, cartilage or bone injury. Examination therefore checks the whole knee, with imaging selected when it can clarify the injury or treatment plan.
What treatment may involve
Immediately after injury, stop sport, avoid twisting and protect the leg from further giving way. A covered cold pack and elevation may reduce discomfort. Use crutches or a brace only with suitable fitting and instructions.
Management considers how much each ligament is damaged, whether the MCL can heal with protection, the degree of instability, associated injury and the activities you want to regain. Physiotherapy develops knee motion, muscle capacity and control. If reconstruction is being considered, prehabilitation may improve readiness. Return to pivoting sport follows functional testing rather than pain relief alone.
When to arrange urgent help
Prompt review is needed when the knee will not straighten, repeatedly buckles, develops marked swelling or cannot take weight. Go to urgent care for visible deformity, an open wound near the joint, a cold or pale foot, new numbness or weakness, or severe symptoms after a forceful collision.
Decisions to discuss with your clinician
Ask whether either tear is partial, which direction remains unstable, whether the meniscus or cartilage is affected, what brace settings or weight-bearing limits apply, and what objective criteria will be used for the next rehabilitation stage.

Questions people ask
Will an MCL heal while an ACL remains torn?
Some MCL injuries can regain useful stability with protection and rehabilitation. The result depends on the tear location and extent, alignment and other damage, so follow-up examination matters.
Is a brace enough for a combined tear?
A brace can protect selected movement directions, but it does not replace muscle rehabilitation or reassessment. Whether it is suitable depends on the precise injury pattern.
Why can the knee feel better but still be unstable?
Pain and swelling may settle before the ligaments and neuromuscular control can manage turning loads. Stability testing and task-based rehabilitation help identify that gap.
