The key point

These ligaments normally work together to control front-to-back movement, rotation and inward opening of the knee. Damage to all three can leave the joint markedly unstable and can occur with injury to cartilage, bone, nerves or blood vessels.

Why combined damage matters

The ACL checks forward movement and rotation of the shin, the PCL checks backward movement, and the MCL reinforces the inner knee. When several restraints fail together, ordinary actions such as turning or stepping down may feel unreliable.

Pain, rapid swelling, bruising, restricted movement and giving way can accompany this injury. Examination establishes the direction of instability. X-rays may check alignment or bone injury, while other imaging may help map ligament and cartilage damage when it will affect care.

Do not test the knee by twisting or repeatedly trying to walk through instability. Keep the leg supported, use a wrapped cold pack for comfort and follow professional advice about a brace, crutches and weight-bearing.

Some ligaments may be managed with structured rehabilitation, while certain unstable combinations may need surgery. Prehabilitation can prepare movement and strength if an operation is planned, and later rehabilitation progresses by function rather than a calendar alone.

Signs that need urgent attention

Seek urgent care if the foot is cold, pale or unusually weak, if sensation changes below the knee, if the joint is visibly deformed, or if severe swelling and pain are worsening. These features can signal injury beyond the ligaments. A knee that cannot support weight or is locked also warrants prompt medical assessment.
