Aching Pain in the Left Testicle and Groin

Discomfort can arise from the testicle or epididymis, an inguinal hernia, the urinary tract, strained groin muscles, or pain referred from the hip or lower back. The location and character of the pain alone cannot confirm the cause.

A gradual ache after running, kicking, lifting, or changing direction may accompany irritation of the adductor muscles or other tissues around the groin. Hip problems can also produce groin discomfort, particularly when pain is triggered by deep bending, twisting, or prolonged sitting.

Scrotal inflammation or infection may cause tenderness and swelling, sometimes with fever, painful urination, or discharge. A groin bulge that becomes more noticeable with coughing or standing can occur with a hernia. A clinician may need to examine the abdomen, groin, hips, and scrotum because these causes can overlap in how they feel.

Pause heavy lifting, running, and movements that clearly worsen the ache. Supportive underwear may reduce pulling on the scrotum. Do not massage a swollen or very tender testicle, and do not try to push back a painful or firm groin lump.

Arrange a medical assessment if the ache persists, returns, or has no clear link to activity. Note when it began, whether onset was sudden or gradual, and whether movement, coughing, urination, ejaculation, or changing position affects it. Also mention swelling, a lump, urinary symptoms, fever, nausea, recent injury, and sexual-health concerns.

Seek emergency care now for sudden or severe testicular pain, especially with nausea, vomiting, marked swelling, or a testicle that appears higher or sits differently. These features can occur when blood flow to a testicle is interrupted, and prompt examination matters even if the pain later eases.

Urgent assessment is also appropriate after significant trauma; for fever with a hot, red, or rapidly swelling scrotum; or for a painful groin bulge that becomes firm, cannot be gently reduced, or occurs with vomiting or abdominal swelling. Visible blood in the urine or inability to pass urine also warrants prompt care.

Ask which structures seem to be producing the pain, whether a urine test or ultrasound is appropriate, and which activities are safe while the cause is being assessed. If the examination suggests a muscle or hip source, ask whether guided rehabilitation could help and which symptoms should stop exercise.
