What Achilles tendinitis means for runners

How the structures involved in Achilles Tendinitis differ from normal

Short answer

In runners, Achilles tendinitis means the tendon linking the calf to the heel has become painful and less tolerant of repeated running load.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Affected structure
The strong tendon connecting the calf muscles to the heel bone
Typical runner complaint
Pain or stiffness around the back of the ankle that responds to running load
Rehabilitation focus
Adjusting aggravating activity and rebuilding calf and tendon capacity progressively

The short answer

It often develops after the tendon is asked to handle more stress than it has adapted to, such as a sharp increase in mileage, faster sessions, hills, or a change in footwear or running surface.

What is happening in the tendon

The Achilles tendon transfers force from the calf muscles to the heel, helping the foot push away from the ground. A runner may notice stiffness on first getting up, tenderness along the tendon or near its heel attachment, and pain that changes with activity. The term Achilles tendinopathy is also used because persistent symptoms can involve changes in tendon structure and load capacity, not inflammation alone.

Symptoms may settle as the tendon warms up yet return later in the run or afterward. This pattern can still signal that the current training demand exceeds what the tendon can comfortably manage.

How to respond to symptoms

Reduce or pause the running that clearly aggravates the pain rather than repeatedly testing it. Lower-impact exercise may help maintain fitness if it does not provoke symptoms. Review recent changes in distance, pace, hills, surfaces, recovery, and shoes to identify a load increase that can be adjusted.

A physiotherapist or sports-medicine clinician can examine the pain location, ankle movement, calf strength, and running demands. Rehabilitation commonly uses progressively loaded calf exercises, with the exercise type and range adapted for pain at the heel attachment or higher along the tendon. Return to running is usually gradual and guided by how the tendon responds during activity and the following day.

When medical assessment matters

Arrange an assessment if pain keeps returning, is worsening, changes your walking or running form, or does not improve after reducing the aggravating load. Examination can distinguish Achilles tendinitis from other causes of heel or ankle pain and help tailor rehabilitation.

Seek urgent care after a sudden pop or snap, especially with abrupt severe pain, swelling, a new gap in the tendon, or difficulty pushing off the foot or rising onto the toes. These signs can occur with an Achilles tendon tear and should not be treated as routine training soreness.

Useful questions at your appointment

Ask where along the tendon the problem is located, whether another cause of heel pain needs consideration, which calf-loading exercises suit that location, and what symptom response is acceptable during rehabilitation. It is also useful to ask which running variables to change first and what signs should delay your next increase in training.

Questions people ask

Can I keep running with Achilles tendinitis?

That depends on symptom severity and the tendon’s response afterward. Continuing unchanged through escalating pain may prolong irritation; a clinician can help set a tolerable running level while rehabilitation rebuilds capacity.

Is Achilles tendinitis the same as an Achilles rupture?

No. Tendinitis or tendinopathy is a load-related tendon problem, while a rupture is a tear. A sudden pop with immediate loss of push-off strength needs urgent assessment for a possible rupture.

Why is the Achilles stiff in the morning?

An irritated or load-sensitive tendon can feel stiff after rest and loosen with movement. Morning stiffness can help track how the tendon is responding to the previous day’s activity.
