The short answer
The upper gap may feel different from the area at or below the umbilicus. It can become clearer when the head lifts from a supported position or when the abdomen handles greater pressure. A gap can be present without pain, while a narrower area can still feel poorly supported.
What an examination looks for
An assessor may feel along the linea alba at rest and during a small abdominal effort. They consider the distance between the rectus muscles, how deeply the fingers sink, whether the midline develops tension, and whether a ridge appears. They may also observe breathing, trunk movement, lifting and any pelvic-floor symptoms.
The purpose is to understand load transfer across the abdominal wall, not to assign a result from a single finger measurement. Ultrasound is not routinely necessary, although a clinician may request imaging if the examination is uncertain or a hernia or another source of symptoms is suspected.
Building strength safely
Begin with movements you can perform without pain, uncontrolled breath-holding or a large increase in doming. Coordinate an exhale with effort, then gradually increase resistance, range and complexity as control improves. The aim is useful strength for tasks such as carrying a baby, lifting groceries, exercising and getting up from the floor.
Postnatal physiotherapy may include abdominal and pelvic-floor assessment, advice on daily loading and a progressive exercise programme. Recovery does not depend on forcing the muscles together. Binding garments may provide temporary support or comfort for some people, but they do not replace progressive muscle training.
When further review is appropriate
Book a clinical review for persistent pain, a distinct lump, worsening symptoms or difficulty returning to normal activity despite modified exercise. Obtain urgent help if a bulge becomes suddenly very painful, firm or discoloured, cannot be gently reduced when relaxed, or occurs with vomiting. That pattern may indicate a hernia complication rather than diastasis alone.

Ask how the midline behaves under load, whether there is evidence of a hernia, and which symptoms should guide exercise modification. Discuss your preferred activities and whether pelvic heaviness, urine leakage, back discomfort or surgical history should shape the rehabilitation plan.

Does the gap need to close completely?
Not necessarily. Better tension, control and function can matter more than complete closure, and a residual gap may not prevent comfortable activity.
Are sit-ups forbidden with diastasis recti?
