Abnormal spotting between periods: what to know

The cause cannot be judged from the amount of blood. Cycle timing, contraception, pregnancy possibility, bleeding after sex, pain, and discharge help a clinician decide which checks are appropriate.

Temporary hormone shifts or hormonal contraception can produce breakthrough bleeding. Other possibilities include cervical irritation, sexually transmitted infection, endometrial polyps, uterine fibroids, and bleeding related to pregnancy. Chlamydia can present with bleeding between periods or after sex. Cervical dysplasia, cervical cancer, and womb cancer can also present with abnormal bleeding; spotting by itself does not establish any of these diagnoses.

After menopause, any vaginal bleeding is unexpected and needs prompt clinical review.

Use a calendar or app to mark bleeding days and describe whether the blood was seen only when wiping or required period protection. Include pelvic discomfort, pain during sex, unusual discharge, urinary symptoms, fever, and any missed contraceptive doses. If pregnancy is possible, take a pregnancy test and share the result. Depending on the history, assessment may include an examination, infection swabs or urine testing, review of cervical screening, blood tests, or pelvic imaging.

Get urgent help if bleeding is heavy, you feel faint or very weak, or you have severe pelvic pain. Possible pregnancy together with bleeding and one-sided pelvic pain, shoulder-tip pain, dizziness, or collapse requires emergency assessment. Seek prompt advice for fever, foul-smelling discharge, or escalating pain. Book a non-emergency visit when spotting repeats, continues, follows sex, or differs clearly from your usual cycle.

You might ask whether the bleeding pattern points to a hormonal, cervical, uterine, infectious, or pregnancy-related cause. Clarify which tests are being recommended, what each test can show, whether current medicines could contribute, and when you should return if the first assessment does not find a cause.
