Abnormal spotting after a period: what to know

A small amount of brown discharge just after menstrual flow may be older blood leaving the uterus. Bleeding that is new for you, returns across cycles, occurs after sex, or comes with pain should be checked rather than assumed to be part of the period.

The colour alone does not identify the cause. Hormonal changes, missed contraceptive doses, starting or changing hormonal contraception, irritation of the cervix, infection, pregnancy-related bleeding, polyps, or fibroids can alter the timing of bleeding. Endometrial polyps and uterine fibroids can present with bleeding between periods. Cervical dysplasia and cancers of the cervix or womb can also present with unexpected bleeding, so ongoing symptoms need evaluation even when the amount seems light.

Bleeding after menopause is not a menstrual period and should be assessed promptly.

Record when the spotting began, its colour and flow, whether it followed sex, and any pelvic pain, discharge, fever, dizziness, or pregnancy possibility. Note recent contraception changes and medicines that affect bleeding. A clinician may discuss a pregnancy test, pelvic examination, infection testing, cervical screening history, blood tests, or ultrasound according to your symptoms and life stage. Treatment depends on the cause; do not stop prescribed contraception or other medicine without clinical advice.

Seek urgent care for heavy bleeding, fainting, marked dizziness, weakness, severe or worsening pelvic pain, shoulder-tip pain, or bleeding with a possible pregnancy. Fever, foul-smelling discharge, or significant pelvic pain also warrants prompt assessment because infection may need treatment. Arrange a routine appointment if spotting keeps returning, changes your usual pattern, happens after sex, or appears after menopause.

Ask which causes fit the timing of your bleeding, whether pregnancy or infection testing is appropriate, and whether your cervix or uterus needs examination or imaging. You can also ask what change in flow, pain, or discharge should make you seek care sooner and how to manage your current contraception while the cause is investigated.
