The short answer

During an abdominoplasty, a surgeon may bring the connective covering over the abdominal muscles toward the midline and remove selected excess skin. The muscles themselves are not routinely cut to correct diastasis. Surgery leaves a scar and requires a protected recovery period.

What the operation may change

Repair can reduce midline bulging and may improve trunk support for selected patients, but it cannot promise relief from back pain, pelvic floor symptoms or every functional concern. Those symptoms may have several causes that need separate assessment. A coexisting hernia can alter the surgical plan.

Abdominoplasty is different from physiotherapy: surgery changes tissue position, while rehabilitation trains breathing, pressure control and coordinated movement. Exercise cannot remove loose skin, yet it may improve function enough that some people choose not to have an operation.

Steps before making a decision

Start with an examination that confirms diastasis recti and checks for hernia. Discuss previous abdominal operations, medicines, smoking or nicotine use, healing problems, pregnancy plans and the practical support available during recovery. Future pregnancy or major weight change can stretch the repair and may affect timing.

A trial of tailored rehabilitation can clarify which limitations respond to improved strength and coordination. If surgery remains appropriate, ask a qualified plastic surgeon where the scar will sit, whether muscle-covering repair and hernia treatment are planned, and what lifting restrictions will apply.

When medical attention is needed

Before surgery, urgent assessment is needed for a suddenly painful hard bulge, vomiting, or inability to pass stool or gas. After an operation, follow the surgeon's instructions and seek prompt help for breathlessness, chest pain, fainting, one-sided calf swelling, heavy bleeding, spreading redness, fever, wound opening or rapidly increasing abdominal swelling.
