Abdominal tear during pregnancy: what it may mean

People may use the phrase for a strained abdominal muscle, stretching or separation of the abdominal muscles, or sudden pain from pregnancy-related changes. A clinician must distinguish these from pregnancy complications or another abdominal problem.

A muscle strain may follow lifting, twisting, coughing or exercise and can hurt more when the muscle contracts. Separation along the midline of the abdomen can produce a ridge or bulge, but it is not the same as an internal organ tearing. Pain can also arise from the uterus and its supporting tissues as the pregnancy changes.

Abdominal swelling has many possible explanations. Its appearance alone cannot identify the cause, so note whether it is new, one-sided, tender, firm or associated with digestive, urinary or pregnancy symptoms.

Contact your maternity team or clinician for new or persistent abdominal pain. Tell them where it started, what you were doing, whether movement changes it, and whether you have bleeding, fluid loss, fever, vomiting, urinary symptoms or a change in the baby's usual movements.

Until you receive advice, avoid the activity that triggered the pain and do not start pain medicine, abdominal binding or strenuous rehabilitation without checking that it is suitable in pregnancy. A physical examination and, when appropriate, pregnancy-safe imaging can clarify the cause.

Seek urgent maternity or emergency assessment for severe or worsening pain, heavy vaginal bleeding, fainting, breathing difficulty, a rigid or very tender abdomen, fluid leaking from the vagina, regular painful tightening, fever with illness, persistent vomiting, or reduced fetal movement after movements have become established.

After a fall, collision or direct blow to the abdomen, contact maternity services promptly even if the discomfort initially seems manageable.

Ask which structure seems to be causing the pain, whether the pregnancy needs monitoring, which movements and pain relief are appropriate, and what specific change should prompt reassessment. If there is a visible bulge, ask whether physiotherapy assessment would help.
