How abdominal separation is checked

Short answer
An abdominal separation test checks the midline gap and tissue tension while the abdominal muscles gently contract.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Test type
- Physical examination during gentle abdominal contraction
- Useful observations
- Gap location, tissue tension, doming, pain and control
- Possible imaging
- Ultrasound when greater measurement detail is clinically useful
The short answer
A clinician usually feels along the line between the breastbone and pubic bone as you lift your head slightly or perform another controlled contraction. The check should consider control, doming, symptoms and daily function, not only the number of finger widths.
How to understand the findings
Diastasis recti involves widening and thinning of the connective tissue between the paired abdominal muscles. The examiner may note where the gap is present, how firm the midline feels under gentle pressure, and whether a ridge appears with effort. Measurements can vary with body position, breathing and how strongly you contract.
A self-check may identify a possible separation but cannot reliably distinguish it from a hernia or explain pain. Ultrasound is sometimes used when a more precise measurement or clarification is clinically useful; it is not required for every person.
What to do after a check
If you try a gentle self-check, lie comfortably with knees bent, relax your abdomen, then lift your head a little while breathing out. Stop if this causes pain, dizziness or marked strain. Avoid repeatedly poking deeply into the midline, because the result depends on technique and is not a diagnosis.
Seek a physiotherapy or medical assessment if the finding comes with pain, troublesome doming, pelvic floor symptoms, or difficulty with lifting and routine movement. The plan may focus on breathing, load management and progressive trunk control rather than closing a gap to a particular width.
When the result needs medical review
Book a routine review for a new persistent bulge, ongoing pain or uncertainty about whether the area is a hernia. Obtain urgent help if a bulge becomes suddenly painful, firm, discoloured or cannot be gently reduced, especially with vomiting, fever, faintness, or inability to pass stool or gas.
Questions to take to an assessment
You might ask what the examiner found about midline tension and control, whether the bulge could be a hernia, which activities are currently appropriate, and how improvement will be judged. Mention pregnancy or birth history, abdominal surgery, pain triggers and pelvic floor symptoms.

Questions people ask
Is a finger-width self-test a diagnosis?
No. Finger size, pressure, position and muscle effort affect the result, and a clinician can also assess tissue tension, function and possible hernia.
Do I need imaging to confirm diastasis recti?
Often a physical examination is enough. A clinician may request ultrasound when the diagnosis is uncertain or a precise measurement would change management.
