Abdominal pain with dark urine: what it may mean

Urine can look darker when it is concentrated, but a tea- or cola-like colour alongside pain may also reflect bile pigment or blood. The location of the pain and other symptoms help guide the next steps.

Pain in the upper right abdomen with yellow eyes or skin, pale stools, nausea, or itching can occur when bile flow is obstructed or the liver is inflamed. Gallstones are one possible cause of blocked bile flow, although symptoms and testing are needed to identify the cause.

Pain in the side or back, burning when urinating, fever, or visible blood can point toward a urinary or kidney problem. Vomiting, diarrhoea, heat exposure, or poor fluid intake can produce concentrated urine through dehydration.

Arrange a same-day clinical assessment if the dark colour persists, the pain is new or worsening, or you notice yellowing, fever, urinary symptoms, or repeated vomiting. A clinician may examine your abdomen and request urine and blood tests; imaging may be chosen according to the findings.

Until assessed, note the urine colour, pain location, when symptoms began, medicines or supplements taken, fluid intake, and any change in stools. Sip fluids if you can keep them down, but do not delay care to see whether drinking alone changes the colour.

Seek urgent medical care for severe or rapidly worsening abdominal pain, confusion, fainting, marked drowsiness, inability to keep fluids down, very little urine, blood in vomit or stool, a swollen tender abdomen, or yellow skin with fever. Sudden side pain with fever or difficulty passing urine also needs urgent assessment.

Ask which organ system the symptom pattern suggests, whether urine and liver or kidney blood tests are appropriate, whether an ultrasound is needed, and which changes should prompt emergency care. Mention any recent infection, travel, strenuous exercise, new medicine, supplement, or pregnancy possibility.
