Abdominal pain and spotting between periods

Light bleeding can occur around ovulation or after hormonal changes, while pain and bleeding may also accompany pregnancy-related problems, infection, ovarian cysts, endometriosis, fibroids, or changes affecting the cervix. The pattern and accompanying symptoms help narrow the possibilities.

Notice where the pain sits, whether it is one-sided, and whether the bleeding is light staining or closer to a period. Also consider missed contraception, a late period, unusual discharge, pain during sex, urinary symptoms, fever, nausea, or bowel changes.

A pregnancy test is relevant whenever pregnancy is possible, even if the bleeding seems period-like. Pain with bleeding in pregnancy needs prompt clinical advice because an ectopic pregnancy or early pregnancy loss must be considered.

Record the bleeding colour and amount, the pain location, when each began, and their relationship to your menstrual cycle. Use pads if this makes the amount easier to monitor. Avoid sex if it worsens pain or bleeding, and do not douche or use vaginal treatments without knowing the cause.

Arrange a routine appointment if spotting recurs, follows sex, or comes with ongoing pelvic discomfort. A clinician may discuss contraception, perform a pregnancy test, check for infection, examine the abdomen or pelvis, or recommend imaging according to your symptoms.

Seek urgent medical care for severe or rapidly worsening pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, breathing difficulty, or a positive pregnancy test with pain or bleeding. Fever, repeated vomiting, or feeling acutely unwell also warrants prompt assessment.

Ask whether the timing suggests ovulation or a contraceptive effect, whether pregnancy and infection testing are appropriate, and what examination or scan might clarify the cause. Mention cervical screening history, recent contraceptive changes, and any medicines that affect bleeding.
