Abdominal pain that radiates to the groin

Pain can be felt away from its source because nearby organs and tissues share nerve pathways. A urinary stone may produce strong flank or abdominal pain that moves downward toward the groin. A groin hernia can cause an ache or bulge that becomes more noticeable with standing, coughing, lifting, or straining.

Pelvic and reproductive causes vary by anatomy. Testicular torsion can present with sudden testicle or groin pain and lower abdominal discomfort. Ovarian problems or a pregnancy outside the uterus may cause one-sided pelvic pain that extends toward the groin. Hip, back, or abdominal-wall injuries can also refer pain into this region.

Notice whether the pain began suddenly or gradually, starts in the flank or pelvis, comes in waves, and changes with movement, coughing, urination, or bowel movements. Check for a groin lump, scrotal swelling, vaginal bleeding, fever, vomiting, urinary discomfort, blood in urine, or pregnancy possibility without repeatedly pressing a painful area.

Arrange an examination for new, recurrent, or unexplained pain even if it eases. Depending on the pattern and anatomy involved, assessment may include the abdomen, groin, back, genitals, or pelvis, together with urine testing, a pregnancy test, blood tests, or imaging. Avoid heavy lifting until a painful bulge or suspected strain has been evaluated.

Go for emergency assessment if there is sudden severe testicle pain, a high or unusually positioned testicle, or scrotal swelling, because delayed treatment can threaten the testicle. A painful groin bulge that becomes firm, changes colour, cannot be gently reduced, or occurs with vomiting also needs immediate care.

Urgent help is necessary for severe pain with fainting, dizziness, heavy vaginal bleeding, or possible pregnancy. Fever or chills with flank pain or urinary symptoms, inability to pass urine, repeated vomiting, a rigid abdomen, or rapidly worsening pain are additional reasons not to wait for a routine appointment.
