Abdominal pain with ovulation

This pain may feel like a twinge, cramp, pressure, or dull ache. Because several pelvic and abdominal conditions can overlap with it, a new, severe, prolonged, or changing symptom should not be assumed to come from ovulation.

The discomfort often sits on one side of the lower abdomen near the middle of the menstrual cycle. It may occur alongside fertile-type cervical mucus or slight spotting. The affected side can change, remain the same for several cycles, or be hard to identify.

Pain can also arise from an ovarian cyst, endometriosis, pelvic infection, appendicitis, bowel irritation, or a urinary problem. If pregnancy is possible, ectopic pregnancy is an urgent consideration when pelvic pain is accompanied by bleeding, dizziness, faintness, or shoulder-tip discomfort.

Rest, gentle movement, and a warm pack may ease mild discomfort. Record the first day of each period, the day pain appears, its location, and any bleeding, discharge, nausea, bowel changes, or urinary symptoms. This shows whether the episodes consistently align with the middle of your cycle.

Before taking a pain medicine, check suitability with a pharmacist or clinician if you could be pregnant, have a history of ulcers or kidney problems, take anticoagulants, or have medicine allergies. Book a non-urgent review when recurring symptoms interfere with normal activities or sex.

Urgent assessment is needed for intense or rapidly worsening pain, fainting, pronounced dizziness, heavy bleeding, fever, repeated vomiting, a swollen or rigid abdomen, or shoulder-tip pain. Do not wait for a predicted ovulation day to pass when these signs occur.

Seek timely clinical review if pain continues, occurs outside the usual cycle pattern, is associated with pain during sex, or comes with unusual vaginal discharge, bowel disturbance, or urinary burning. Testing depends on the history and may include a pregnancy test, urine testing, pelvic examination, or imaging.
