Causes of abdominal pain during ovulation

Short answer
Ovulation can cause brief one-sided lower abdominal pain, but severe, persistent, or unusual pain needs medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical location
- Lower abdomen or pelvis, often on one side
- Helpful record
- Cycle timing, pain location, bleeding, discharge, and accompanying symptoms
- Urgent concern
- Severe pain, fainting, heavy bleeding, fever, or repeated vomiting
The short answer
The discomfort is sometimes called mittelschmerz. It may occur when the ovarian follicle stretches or releases an egg, and a small amount of fluid or blood irritates nearby tissue.
What the pain may mean
Ovulation pain is usually felt low in the abdomen or pelvis and may switch sides between cycles. A dull ache, sharp twinge, or mild cramping around the middle of a menstrual cycle can fit this pattern. Light spotting or clear, stretchy vaginal discharge may occur at the same time.
Timing alone cannot confirm the cause. Ovarian cysts, endometriosis, pelvic infection, bowel problems, urinary conditions, appendicitis, or pregnancy-related complications can also cause abdominal or pelvic pain. Symptoms such as vomiting, fever, unusual discharge, urinary burning, bowel changes, or a missed period help a clinician narrow the possibilities.
What you can do
Pause strenuous activity, rest, and try gentle warmth over the lower abdomen if the discomfort is mild. Keep a cycle diary noting the side, character, triggers, associated bleeding or discharge, and when the pain begins and ends. A repeating mid-cycle pattern gives your clinician useful context.
If you need pain relief, ask a pharmacist or clinician which option suits you, especially if pregnancy is possible or you have kidney disease, stomach ulcers, take blood thinners, or use other medicines. Arrange a routine appointment when pain repeatedly disrupts sleep, work, exercise, or sex.
When to seek medical care
Seek urgent care for sudden severe pain, fainting, marked dizziness, a rigid or very tender abdomen, heavy vaginal bleeding, repeated vomiting, fever, shoulder-tip pain, or pain with difficulty breathing. These features require prompt assessment regardless of where you are in your cycle.
Take a pregnancy test and obtain urgent medical advice if pregnancy is possible and pelvic pain occurs with bleeding, weakness, dizziness, or shoulder pain. Contact a clinician soon if the discomfort lasts beyond the expected ovulation window, becomes progressively worse, or feels different from your usual cycle symptoms.
Questions for your clinician
You could ask whether the timing and location fit ovulation pain, whether a pregnancy test, pelvic examination, urine test, or ultrasound is appropriate, and what changes should trigger urgent review. Also discuss contraception, fertility plans, cycle irregularity, and any previous ovarian cysts or pelvic conditions.

Questions people ask
Can ovulation pain occur on a different side each month?
Yes. The side can vary because ovulation may occur from either ovary, although side changes do not have to follow a predictable pattern.
How can I tell ovulation pain from another cause?
A mild, short-lived, one-sided pain near mid-cycle can suggest ovulation, but timing cannot diagnose it. Persistent pain or additional symptoms warrant clinical assessment.
