Abdominal pain one week after a period

Mild, brief discomfort without other symptoms can often be monitored. New, recurring or worsening pain needs clinical review, especially when there is unusual bleeding, discharge, fever, urinary trouble or a possibility of pregnancy.

Ovulation can cause short-lived lower abdominal discomfort on one side, but cycle timing varies and should not be used as a diagnosis. Cramping with bloating, constipation or diarrhoea may arise from the bowel. Burning urination, urgency or back pain suggests that the urinary tract should be checked.

Pelvic conditions can also cause pain between periods. A clinician may consider an ovarian cyst, endometriosis, infection or another gynaecological cause according to the examination and symptom pattern. Upper abdominal burning, reflux, vomiting or meal-related symptoms may be separate from the menstrual cycle and point toward the digestive system.

Record where the pain sits, whether it is constant or comes in waves, its relationship to meals and bowel movements, and any bleeding, discharge or urinary symptoms. Note the first day of recent periods and whether pregnancy is possible. This pattern is more useful to a clinician than the calendar date alone.

Rest from activities that aggravate the discomfort, drink normally and use a warm pack if it is soothing. A pharmacist can advise on pain relief that is suitable if pregnancy is possible or if you have stomach, kidney or bleeding concerns. Book an appointment if the pain recurs at similar points in the cycle, interferes with daily life or does not settle.

Get urgent medical help for sudden severe pain, fainting, marked weakness, shoulder-tip pain, heavy vaginal bleeding, a rigid abdomen or repeated vomiting. If pregnancy is possible, abdominal or pelvic pain with bleeding, dizziness or shoulder-tip pain requires urgent assessment, even if bleeding seemed like a period.

Arrange timely medical care for fever, offensive-smelling discharge, pain during sex, burning urination, blood in urine or stool, worsening one-sided pain, or persistent changes in bowel habits. These accompanying features can guide examination, pregnancy testing, urine testing or other investigations.

Ask whether the location and cycle pattern suggest a pelvic, urinary or bowel source; whether a pregnancy or urine test is appropriate; and which changes should trigger urgent reassessment. Mention contraception, recent sexual health concerns, previous cysts or endometriosis, and all medicines you use.
