What an abdominal muscle tear is
Tears range from a small partial injury to extensive disruption. The label alone does not determine treatment: function, examination findings, the injury mechanism, and whether another abdominal condition is possible all shape the next step.
How the injury may feel
The injury may happen during sprinting, forceful throwing, heavy lifting, a sudden twist, or an intense cough. Pain is often reproduced when the abdominal wall tightens, such as while sitting up, laughing, sneezing, or resisting trunk rotation. Local tenderness and bruising can accompany a larger tear, but their absence does not rule out a smaller one.

A protected cold pack may help comfort soon after injury; do not apply ice directly to skin or use reduced sensation as permission to exercise harder.
Rehabilitation usually builds from comfortable breathing and gentle isometric contraction toward trunk control, limb loading, rotation, and the specific demands of work or sport. A clinician can guide the pace when there is clear weakness, substantial bruising, a high-demand return goal, or uncertainty about how much tissue is involved.
When assessment should not wait
Get prompt medical advice for a new abdominal bulge, spreading bruising, marked weakness, severe pain, inability to stand upright, or symptoms that continue to worsen. An evaluation is also sensible when pain fails to improve, repeatedly interrupts sleep, or returns each time ordinary activity resumes.
Seek urgent care if abdominal pain follows a strong blow or occurs with faintness, breathlessness, chest discomfort, fever, repeated vomiting, blood in vomit or stool, or a rigid or increasingly swollen abdomen. These warning signs require assessment for injuries and illnesses beyond the muscle wall.
