Fever for five days followed by a rash

Some viral illnesses produce a rash as the fever settles, while other infections cause fever and rash together. A rash may also be a reaction to a medicine started during the illness. Its location, colour, texture, spread, whether it blanches when pressed, and whether it is painful, itchy, blistering, or bruiselike all help distinguish possible causes.

A clinician will also consider age, vaccination history, recent travel, sick contacts, new medicines, insect exposure, sore throat, cough, red eyes, mouth changes, swollen glands, and joint symptoms. The pattern is informative, but no single timing feature establishes a diagnosis.

Arrange a same-day clinical review, particularly if fever is continuing. Take clear photos as the rash changes, record temperatures and medicines, and bring the medicine packaging to the appointment. Encourage fluids and avoid overheating. Do not apply a new medicated cream or stop an essential prescription unless a clinician advises it, except when emergency allergy symptoms require immediate help.

Limit close contact with pregnant people, newborns, and anyone with reduced immunity until a clinician has considered whether the illness could spread. Do not share towels, cups, or utensils.

Call emergency services for trouble breathing, swelling of the lips or tongue, collapse, confusion, a seizure, severe drowsiness, a stiff neck, or a purple or bruiselike rash that does not fade under firm pressure. Rapidly spreading skin pain, blistering, peeling, sores in the mouth or eyes, or a very unwell appearance also require urgent care.

Prompt assessment is especially important when the patient is a young infant, pregnant, immunocompromised, unable to drink, passing much less urine, or developing persistent abdominal pain, severe headache, or worsening fever.

Ask whether the rash pattern suggests an infection, medicine reaction, or another cause; whether testing or isolation is needed; which treatments are safe for itching or fever; and what change in the rash should prompt emergency reassessment.
