What the spotting may mean
Small amounts of blood may come from changes where the pregnancy attaches in the uterus or from a cervix that bleeds more easily during pregnancy, sometimes after sex or an examination. Spotting can also occur with an early pregnancy loss or an ectopic pregnancy, where the pregnancy develops outside the uterus. Colour and quantity cannot reliably separate these causes, and some people have no pain at first.
Bleeding that seems like a period can make pregnancy dating uncertain. A clinician may use your symptoms, examination, pregnancy-hormone blood tests, and an ultrasound when it is likely to be informative. At this early stage, one scan may not provide a definite answer, so follow-up testing may be recommended.
What to do now
Use a sanitary pad or liner rather than a tampon so you can track the bleeding. Note whether it is only on tissue, reaches the pad, becomes heavier, contains clots or tissue, or comes with pain. Avoid putting anything into the vagina while bleeding, and follow your clinician’s advice about sex. Rest if you feel unwell; routine bed rest is not generally recommended.
Tell the clinician the first day of your last period, any pregnancy-test results, the bleeding pattern, pain location, and whether you have previously had an ectopic pregnancy or fallopian-tube surgery. Do not stop prescribed medicine on your own; ask whether any medicine needs adjustment.
When to seek urgent care
Go to an emergency department now for heavy or rapidly increasing bleeding, severe abdominal or pelvic pain, pain mainly on one side, shoulder-tip pain, fainting, marked dizziness, weakness, breathing difficulty, or feeling acutely unwell. These symptoms can occur with significant blood loss or an ectopic pregnancy and should not wait for a routine appointment.
Seek prompt medical advice if spotting persists, repeatedly returns, develops an unpleasant smell, or is accompanied by fever, chills, or worsening cramps. Even when the spotting is light and occurs without pain, let your maternity clinician know so they can decide whether monitoring or assessment is appropriate.
