Temporary fussiness, cluster feeding, distraction at the breast, or waking more often can occur without a supply problem. Persistent poor weight gain, fewer wet nappies than usual, or weak feeding deserves assessment.

Steps that support milk production

Offer the breast responsively and allow active feeding before switching sides. Skin-to-skin contact and frequent, effective milk removal signal the breasts to make milk. If feeds are painful, nipples look pinched afterwards, or swallowing is limited, arrange for a lactation professional to observe an entire feed.

If supplementation is medically advised, ask how to protect supply with expressing and how the baby's intake will be reviewed. Do not start herbal products or prescription milk-boosting medicines without a clinical assessment because they can have side effects and may not address poor milk transfer.

Signs that need prompt help

Get same-day medical advice if your baby is unusually sleepy, difficult to wake for feeds, repeatedly refuses feeds, has a dry mouth, passes much less urine than usual, or has dark urine. These may indicate inadequate intake or illness and should not be managed only by trying to increase supply at home.

Seek care for yourself if you have a hot, red, painful breast with fever or feel acutely unwell. Breast inflammation or infection can affect feeding and may require treatment while breastfeeding continues.

What to ask at a feeding review

Ask whether your baby's growth pattern and hydration are reassuring, whether attachment and swallowing are effective, and whether tongue movement or another health issue is interfering with feeding. Also review your medicines, recent illness, breast surgery, pumping routine, and any long gaps between milk removals.
