Short answer

At 20 weeks pregnant, heartburn commonly reflects acid moving into the oesophagus as hormones relax digestive muscles and the growing uterus changes abdominal pressure.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What to know now

You may notice a hot or burning feeling in the chest, sour-tasting fluid, burping, or discomfort after meals. These features fit reflux, but chest or upper abdominal pain with warning signs should not be managed as routine heartburn.

How pregnancy affects reflux

A ring of muscle normally closes after food enters the stomach. Pregnancy-related hormonal changes can make that ring looser, allowing stomach contents to travel back towards the throat.

The uterus also occupies more space as pregnancy progresses. A large meal, bending at the waist, or lying flat can then make symptoms easier to provoke. Heartburn may come and go and can vary with foods, meal size, and time of day.

A practical meal-timing plan

Divide food into smaller meals and snacks rather than filling the stomach at once. Eat without rushing, limit drinks that reliably trigger burning, and give yourself time upright before bed. If a particular food repeatedly causes symptoms, reduce it while keeping your overall pregnancy diet varied.

At night, elevate the torso with a wedge or by safely raising the head end of the sleeping surface. Avoid tight waistbands. If these changes are insufficient, a clinician or pharmacist can help select an antacid or acid-reducing treatment that suits pregnancy and does not interfere with your prescribed medicines or supplements.

Symptoms that need assessment

Call emergency services for crushing or intense chest pain, breathlessness, collapse, clammy sweating, or pain that travels into the jaw, neck, back, or arm. These features require urgent evaluation even if you also have a history of reflux.

Contact your maternity service without delay for severe upper abdominal pain accompanied by headache, visual disturbance, or sudden swelling. Prompt advice is also appropriate for blood in vomit, black stools, repeated vomiting, difficulty swallowing, or inability to drink. Mention ongoing night symptoms at your next maternity or GP visit so treatment can be adjusted.

What to discuss with your clinician

Bring the names of every medicine, supplement, and antacid you use. Ask whether the timing of reflux treatment should differ from iron, whether your symptoms suggest another digestive problem, and what change would warrant an earlier reassessment.

Questions people ask

Is heartburn expected halfway through pregnancy?

It can occur around this stage because hormonal relaxation and changing abdominal pressure promote reflux. Symptoms that are severe, persistent, or unlike your usual burning should be assessed.

Should I stop eating acidic or spicy food?

Only foods that consistently trigger your symptoms need limiting. Keeping a brief record of meals and symptoms can identify patterns without removing foods unnecessarily.

Will milk settle pregnancy heartburn?

Milk may feel soothing briefly for some people, but it does not prevent reflux and may trigger symptoms in others. Smaller meals and remaining upright address common mechanical triggers more directly.
