Spotting two days after sex

The timing alone cannot identify the cause. Consider whether the blood is coming from the vagina rather than the urine or rectum, and whether it coincides with an expected period, a new contraceptive method or vaginal dryness.

Friction can irritate delicate vaginal or cervical tissue, especially with dryness. Cervical inflammation and infections such as chlamydia can also present with bleeding after sex, sometimes with unusual discharge, pelvic discomfort or pain when urinating. Polyps, hormonal changes and fibroids are other possible explanations for bleeding between periods.

Cervical dysplasia and cervical cancer can present with bleeding after sex, while womb cancer can present with unexpected vaginal bleeding, particularly after menopause. These possibilities require examination rather than assumptions based on the amount or colour of blood.

Use a pad or liner to monitor the bleeding, and record when it started, its amount and colour, whether it followed penetrative sex, and any pain, discharge, odour or urinary symptoms. Avoid sex if it worsens pain or bleeding.

Arrange a clinical appointment if spotting returns, persists, occurs after menopause or has no clear explanation. A clinician may discuss pregnancy possibility and contraception, examine the vagina and cervix, and offer pregnancy, infection or cervical screening tests when appropriate.

Seek urgent medical care for heavy bleeding, severe or increasing pelvic pain, fainting, marked dizziness, weakness, shoulder-tip pain or a positive pregnancy test with bleeding. Fever, worsening lower abdominal pain or foul-smelling discharge also needs prompt assessment because infection may require treatment.

Call emergency services if you feel very unwell or may collapse. Urgency depends on the bleeding and accompanying symptoms, not on whether a serious cause is common.

Ask whether you need a pregnancy test, infection testing, a cervical examination or an update to cervical screening. Mention your last period, menopausal status, contraception, medicines and whether bleeding happens every time after sex. Ask when to return if the examination and initial tests do not find a cause.
