Understanding a missed miscarriage at 12 weeks

It is usually identified on ultrasound. Clinicians may confirm measurements carefully or arrange a repeat scan when dates or findings are uncertain. Once confirmed, the main management choices are waiting for tissue to pass naturally, using medicine, or having a procedure.

Some people still feel pregnant because hormone levels do not fall immediately; others notice that nausea or breast tenderness has eased. A change in symptoms does not establish whether a pregnancy is continuing, and missed miscarriage can occur without outward signs.

The news can be shocking, especially after a routine scan with no warning symptoms. Grief, numbness, anger and difficulty concentrating can all occur. Support may come from a partner, trusted person, counsellor, pregnancy-loss service or faith community according to your preference.

Expectant management allows the body to pass the tissue without a planned medicine or procedure. Medical management uses prescribed medication to start or support that process. Surgical management removes pregnancy tissue through the cervix and may be recommended for heavy bleeding, infection, clinical urgency or personal preference.

Discuss what bleeding and cramping to expect, suitable pain relief, where the care will happen, and how completion will be checked. Use pads rather than estimating blood loss from appearance alone, and follow the service's advice about bathing, sex and returning to activity.

Seek urgent assessment for bleeding that rapidly soaks pads, severe or increasing pain, fainting, marked dizziness, fever, chills, foul-smelling vaginal discharge, breathlessness or feeling very unwell. These symptoms can signal substantial blood loss, infection or another complication.

Contact the treating service if bleeding or pain does not follow the pattern they described, or if pregnancy symptoms persist after expected follow-up. Immediate emotional help is also appropriate if grief brings thoughts of self-harm or makes you feel unsafe.

Ask how certain the ultrasound diagnosis is, which options are suitable for your health, how each choice may affect timing and bleeding, and when follow-up is planned. You may also want to discuss blood-group testing, tissue examination when applicable, emotional support and planning for a future pregnancy.
